چکیده
A 33-year-old hypertensive, diabetic woman had a 6 year history of progressive lower extremity weakn ess . More recently she had developed dysesthetic pains in both lower extremities associated with urinary and fecal incontinence. Physical examination revealed a grossly obese female weighing 174 kg. Normal motor and sensory fu nction was noted in the upper extremi ties, but fl accid paralysis and areflex ia were evident in both lower extremities. Th ere was also fascicu lation of the quadriceps femoris muscle bod ies in response to direct percussion. Propri-

Anesthetic management of the morbidly obese parturient.
PURPOSE OF REVIEW Regional analgesia for labor pain and general anesthesia for cesarean section in the morbidly obese parturient is associated with increased maternal and perinatal complications. The purpose of this review is to describe the anesthetic management of the morbidly obese parturient. RECENT FINDINGS Compared to the lean parturient, the morbidly obese parturient has an increased l...
متن کاملReal-time Ultrasound Guided Epidural Catheter Placement in Morbidly Obese Parturients after Unsuccessful Conventional Attempts
Several studies have emphasized the usefulness of preprocedural Ultrasound (US) assessment of lumbar spine. Despite those helpful informations this procedure may remain challenging. This report evaluates the effectiveness of a Real Time US Guided Epidural Anesthesia (RTUGEA) in morbidly obese patients as usual loss of resistance technique combined with a preinsertion US assessment of lumbar spi...
متن کاملEpidural Catheter Placement in Morbidly Obese Parturients with the Use of an Epidural Depth Equation prior to Ultrasound Visualization
BACKGROUND Previously, Balki determined the Pearson correlation coefficient with the use of ultrasound (US) was 0.85 in morbidly obese parturients. We aimed to determine if the use of the epidural depth equation (EDE) in conjunction with US can provide better clinical correlation in estimating the distance from the skin to the epidural space in morbidly obese parturients. METHODS One hundred ...
متن کاملDose adjustment of anaesthetics in the morbidly obese.
Anaesthesiologists must be prepared to deal with pharmacokinetic and pharmacodynamic (PD) differences in morbidly obese individuals. As drug administration based on total body weight can result in overdose, weight-based dosing scalars must be considered. Conversely, administration of drugs based on ideal body weight can result in a sub-therapeutic dose. Changes in cardiac output and alterations...
متن کاملEndotoxin increase after fat overload is related to postprandial hypertriglyceridemia in morbidly obese patients.
The low-grade inflammation observed in obesity has been associated with a high-fat diet, though this relation is not fully understood. Bacterial endotoxin, produced by gut microbiota, may be the linking factor. However, this has not been confirmed in obese patients. To study the relationship between a high-fat diet and bacterial endotoxin, we analyzed postprandial endotoxemia in morbidly obese ...
